Acknowledgement of Risk

Participation in dance involves inherent risks that cannot be completely eliminated. These risks include, but are not limited to, sprains, strains, fractures, falls, collisions, overuse injuries, aggravation of pre-existing medical conditions, cardiovascular strain, fatigue, and other injuries that may result from physical activity.

By participating, I acknowledge that I understand these risks and agree to participate voluntarily. I am responsible for monitoring my own physical condition, participating within my personal limits, and consulting a medical professional if I have concerns about my ability to safely participate.

Release of Liability

In consideration of being permitted to participate in this dance event, I voluntarily assume all risks associated with my participation.

On behalf of myself, my heirs, assigns, and personal representatives, I release, waive, and hold harmless BELLYham, a sole proprietorship owned and operated by Jessica Crockett, its instructors, volunteers, agents, representatives, and the event venue (collectively, the "Releasees") from any claims, liabilities, damages, or expenses arising from injury, illness, death, or property damage related to my participation, including those resulting from the ordinary negligence of the Releasees, to the fullest extent permitted by Washington law.

I also agree to indemnify and hold the Releasees harmless from claims arising from my participation or my breach of this agreement.

Medical Treatment

I authorize emergency medical treatment if necessary and accept full responsibility for any resulting medical expenses.

Personal Property

I understand that BELLYham is not responsible for the loss, theft, or damage of any personal belongings brought to classes, workshops, performances, or events.

Photo & Video Release

I grant BELLYham and its authorized representatives permission to photograph and/or record me during classes, workshops, performances, and events, and to use these images or recordings for promotional, educational, marketing, and social media purposes without compensation. If I do not wish to be photographed or recorded, I will notify BELLYham in writing prior to the event.

Severability

If any provision of this agreement is determined to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

Governing Law

This agreement shall be governed by and construed in accordance with the laws of the State of Washington.

I have read and understand this Acknowledgement of Risk & Release of Liability and sign it voluntarily.

Parent/Guardian Consent (Required for Participants Under 18)

I am the parent or legal guardian of the above-named minor. I have read and understand this Acknowledgement of Risk & Release of Liability, consent to the minor's participation, and agree to all terms of this agreement on the minor's behalf. I also agree to indemnify and hold harmless BELLYham, its owner, instructors, volunteers, agents, representatives, and the event venue from any claims arising from the minor's participation, to the extent permitted by law.
